Home Care Services are available from Partners in Senior Care. This allows our clients to age in the place where they feel most comfortable and secure—Home. Trained caregivers under the supervision of both Geriatric Care Managers and Registered Nurses are available to help the client with activities of daily living. These activities can include, but are not limited to:
Companionship
Bathing
Grooming
Transportation
Assistance with ambulation and exercises
Meal planning and preparation
Light housekeeping
Medication reminders
All of our caregivers are carefully screened before they are hired, including criminal background and reference checks. They are oriented to our client centered philosophy of care and continue to receive additional training in the unique needs of the senior population. They are bonded and insured.
When our Home Care services are used, a Professional Geriatric Care Manger continues to supervise the caregivers to ensure that the Plan of Care is followed. Following home visits, the Care Manager updates and adjusts the plan as necessary. As appropriate, the Care Manager communicates these changes to the client’s family or representative.
